I found the coolest online tool ever–a Logic Model Builder! A logic model is a visual representation, map or flow chart of your program. You show resources, staff, activities, tools and programming, outputs and outcomes in a way that shows how they all flow together.
This one is created for your grant writing pleasure by the US Department of Health and Human Services (DHHS) specifically for child abuse and neglect and post-adoption programs. http://toolkit.childwelfare.gov/toolkit/home.do
It is very spiffy. You type in your information and then choose pre-selected outcomes (a wide range, don’t fret) and then they offer you indicators that come out of those outcomes and then you choose from an approved list of tools to track the indicators. They include really commonly used assessment surveys that even I have heard of, so it’s nothing outlandish that your agency won’t have.
I haven’t found such a tool for other programs, but here are some good general sites for understanding and building logic models:
